Part I: Identification

Information in this section will be used to identify the library and to ensure that the data reported for the library is listed correctly in Kansas Public Library Services

Line 1.3 CITY OR TOWN OF ADMINISTRATIVE ENTITY.
Provide the city or town in which your library is located.

Line 1.4 NAME OF LIBRARY.
Provide the official name of your library.

Line 1.5a STREET ADDRESS.
Provide street address. No post office box numbers because this will be used for geography.

Line 1.6a LIBRARY DIRECTOR'S NAME.
This is the name of the person hired to be responsible for operating the library.

Line 1.6b LIBRARY DIRECTOR'S E-MAIL ADDRESS.
This element is the e-mail address of the director of the administrative entity.

Line 1.7b LIBRARY DIRECTOR'S FAX.
This element is the fax number of the director of the administrative entity.

Part II: General Information

Line 2.1 SQUARE FOOTAGE OF LIBRARY.
Provide the area in square feet, of the main public library. This is the area on all floors enclosed by the outer walls of the library. Include all areas occupied by the library, including those areas off-limits to the public. Include any areas shared with another agency or agencies if the library has use of that area.

Line 2.2 LIBRARY BRANCHES.
List all branches, if any. Include name of branch, branch head librarian, street address (no post office box numbers), city, zip + four, email address, telephone, fax and web address. A branch library is an auxiliary unit of an administrative entity which has at least all of the following: 1) separate quarters, 2) an organized collection of library materials, 3) paid staff, and 4) regularly scheduled hours for opening to the public.

Line 2.2a SQUARE FOOTAGE OF BRANCHES.
Provide the area, in square feet, of the public library branches. Report the total area in square feet for each library branch separately. This is the area on all floors enclosed by the outer walls of the library outlet. Include all areas occupied by the library outlet, including those areas off-limits to the public. Include an areas shared with another agency or agencies if the outlet has use of that area.

Lines 2.2b-h. CONTACT INFORMATION.
These questions will ask the name of librarian, street address, city, zip+four, email address and fax number.

Line 2.3 NUMBER OF BOOKMOBILES IN USE.
These are trucks or vans specially equipped to carry books and other library materials; they serve as traveling branch libraries. Count the number of vehicles in use rather than the number of stops the vehicle makes. Do not count system book vans or courier vans.

Line 2.4 ONLINE PUBLIC ACCESS CATALOG IN USE IN YOUR LIBRARY.
This information about the automated circulation system in use in your library is based on the previous year's report. If this is no longer correct, enter new information on this line.

Line 2.5 CENTRAL LIBRARY.
Mark a check in the checkbox if library is a central library as opposed to the branch of a library.

LINE 2.6 LEGAL SERVICE AREA.
If your library service area has increased during the reporting year, answer yes in the drop box. If no change, answer no.


Part III: Paid Staff (IN FULL-TIME EQUIVALENTS)

Report in full-time equivalents, not number of individual employees. Full-time equivalent means total number of staff hours divided by 40 hours. Two people working 20 hours a week each equals 1 full-time equivalent: (20 + 20) / 40 = 1. Three staff people working 20 hours a week each equal 1.5 full-time equivalent: (20 + 20 + 20) / 40 = 1.5 full-time equivalent.

Line 3.1 HOURS WORKED BY ALA-MLS LIBRARIANS.
Report only full-time equivalents (FTE). DO NOT REPORT NUMBERS OF INDIVIDUAL EMPLOYEES, EITHER FULL-TIME OR PART-TIME. To compute full-time equivalents (FTE) divide the number of hours worked per week by all ALA-MLS employees by 40. When figuring full-time equivalents (FTE), carry your figure to two decimal places, but no more than two.

Line 3.2 HOURS WORKED BY ALL STAFF PERFORMING LIBRARY TASKINGS (Include ALA-MLS).
Report only full-time equivalents (FTE). DO NOT REPORT NUMBERS OF INDIVIDUAL EMPLOYEES, EITHER FULL-TIME OR PART-TIME. To compute full-time equivalents (FTE) divide the number of hours worked per week by all staff performing library tasks, including ALA-MLS librarians, by 40. When figuring full-time equivalents (FTE), carry your figure to two decimal places, but no more than two.

Line 3.3 HOURS WORKED BY ALL OTHER PAID STAFF.
Report only full-time equivalents (FTE) DO NOT REPORT NUMBERS OF INDIVIDUAL EMPLOYEES, EITHER FULL-TIME OR PART-TIME. PART IV: SALARY SURVEY

For the purposes of this report, full-time equals 40 hours per week.

Line 4a. HAS STAFF AT YOUR LIBRARY HAD A SALARY INCREASE DURING THE PAST YEAR?
If your staff received a salary increase, answer yes in the drop box. If not, answer no.

Line 4b. BEGINNING SALARY FOR EMPLOYEES WITH MLS DEGREES, BUT WITHOUT PREVIOUS PROFESSIONAL EXPERIENCE.
This includes only salary for staff with a master's degree in library science with no previous experience as an MLS librarian. Report salary only, no benefits are included.

Line 6.1 CITY MILL LEVY.
This includes all tax and non-tax receipts designated by the city and available for expenditure by the public library. Do not include delinquent or back taxes, carryover funds, fines, fees, or the value of contributed or in-kind services or gifts or donations.

Line 6.2 DISTRICT MILL LEVY.
This includes all tax and non-tax receipts designated by library district and available for expenditure by the public library. Do not include delinquent or back taxes, carryover funds, fines, fees, or the value of contributed or in-kind services or gifts or donation.

Line 6.3 TOWNSHIP LIBRARY MILL LEVY.
This includes all tax and non-tax receipts designated by township and available for expenditure by the public library. Do not include delinquent or back taxes, carryover funds, fines, fees, or the value of contributed or in-kind services or gifts or donations.

Line 6.4 COUNTY MILL LEVY.
This includes all tax and non-tax receipts designated by the county and available for expenditure by the public library. Do not include delinquent or back taxes, carryover funds, fines, fees, or the value of contributed or in-kind services or gifts or donations.

Line 6.5 MOTOR VEHICLE TAX.
Include payments received from any local government entity from motor vehicle funds. This includes RV, 16-20 M, boat and aircraft taxes. If none, enter "0".

Line 6.6 LAVTR.
Include payment received from any local government entity from local ad valorem tax reduction. (LAVTR) funds, if any. If none, enter "0".

Line 6.7 EMPLOYMENT BENEFITS FUND.
Include any payments received for a separate library employee benefits fund. If your library does not have a separate library employee benefits fund, enter "0".

Line 6.8 INDIRECT LOCAL PUBLIC SUPPORT.
(Additional municipal government funds or services) Include the monetary value of any local government contributions, such as rent, custodial services, utilities, etc. If none, enter "0".

Line 6.9 TOTAL LOCAL INCOME.
Sum of Lines 6.1b + 6.2b + 6.3b + 6.4b + 6.5 + 6.6 + 6.7 + 6.8

Line 6.10 STATE GRANT-IN-AID.
Include your past year State Aid grant amount.

Include your library's past year system grant, if any. If none, enter "0".

Line 6.12 LSTA GRANT(S), IF ANY.
Include any grants passed through the State Library from the Library Services and Technology Act (LSTA).

Line 6.13 OTHER INCOME.
Report all income other than that given in 6.1a-6.12. Do not include income designated for capital purposes; the value of any contributed or in-kind services or non-monetary gifts or donations; or carryover funds from the previous year.

PART VII: FINANCIAL INFORMATION Ð EXPENDITURES

Line 7.1 SALARIES AND WAGES.
Include salaries and wages before deductions for all staff paid from the library budget for the past year. Exclude employee benefits.

Line 7.2 EMPLOYEE BENEFITS.
Include benefits paid to all employees from the library budget, including a separate employee benefits fund if your library has one. Examples: Social Security, retirement, medical insurance, life insurance, guaranteed disability income protection, unemployment compensation, worker's compensation, and tuition.

Line 7.3 TOTAL STAFF EXPENDITURES.
Sum of Lines 7.1 and 7.2.

Line 7.4a EXPENDITURES ON PRINT MATERIALS.
Include expenditures for government documents, pamphlets, paperbacks. Exclude expenditures for periodicals, microforms, or book processing.

Line 7.4b EXPENDITURES ON PERIODICALS.
Report the amount spent for current periodical subscriptions during the past year. Exclude expenditures for microforms or binding of periodicals.

Line 7.4c TOTAL EXPENDITURES ON PRINT MATERIALS.
Sum of Lines 7.4a and 7.4b.

Line 7.5 EXPENDITURES ON ELECTRONIC MATERIALS.
(Software, CD-ROMs) Report expenditures for materials considered part of the collection, whether purchased or leased, that are digital or designed to be processed by computer. Examples: CD-ROMS, U.S. Census tapes, locally-mounted databases, serials, and reference tools. Include expenditures for equipment only when the cost is inseparably bundled into the price of the information service product. Exclude expenditures for library system software and microcomputer software used only by the library staff.

Line 7.6 EXPENDITURE ON OTHER MATERIALS.
Report total expenditures for materials not reported on Lines 7.4-7.5. Examples: audio CDs, videos, audio cassettes, films, slides, and microforms.

Line 7.7 TOTAL COLLECTION EXPENDITURES.
Sum of Lines 7.4c-7.6.

Line 7.8 EXPENDITURES ON E-BOOKS, DATABASES, CURRENT ELECTRONIC SERIALS (Online Expenditures).
Report all expenditures from the library budget associated with access to electronic materials and services. Include expenditures fro computer hardware and software, whether purchased or leased; services provided by bibliographic utilities, networks, consortia, and commercial services; maintenance; and equipment used to run information service products when the cost can be separated.

Line 7.9 ALL OTHER OPERATING EXPENDITURES.
Report the total of all other operating expenditures not reported in Lines 7.1-7.8.

Line 7.10 OTHER OPERATING EXPENDITURES.
Sum of Lines 7.8-7.9.

Line 7.11 TOTAL OPERATING EXPENDITURES.

PART VIII: FINANCIAL INFORMATION Ð CAPITAL FUNDS

Funds designated for the acquisition of or additions to fixed assets such as building sites, new buildings and building additions, new equipment, initial book stock, furnishings for new or expanded buildings, and new vehicles. This excludes replacement and repair of existing furniture and equipment, regular purchase of library materials, and investments for capital appreciation.

Line 8.1 DOES YOUR LIBRARY HAVE A CAPITAL IMPROVEMENT FUND BASED ON K.S.A. 12-1258?
Choose "yes" only if your library has a capital improvement fund based on the statute K.S.A. 12-1258 (See below). If your fund is not established under the statute, choose "no". Although your library may have a capital improvement fund, choose "yes" only if it is based on the statute.

K.S.A. 12-1258. "Public libraries; capital improvement fund. The library board of any public library is hereby authorized to direct a transfer annually from the general operating fund of such library not to exceed 10% of the amount of money credited to such fund to a capital improvement fund. All money credited to such fund shall be used by the library board for the purpose of improving, furnishing, equipping, remodeling or making additions to the library. Such fund shall not be subject to the provisions of K.S.A. 79-2925 to 79-2937 and amendments thereto. If the library board determines that money which has been transferred to such fund or any part thereof is not needed for the purpose for which transferred, the library board is hereby authorized to direct a retransfer of such amount not needed to the general operating fund and such retransfer and expenditure thereof shall be subject to the provisions of K.S.A. 79-2925 to 79-2937 and amendments thereto.

In making the budget of the library, the amounts credited to, and the amount on hand in, the capital improvement fund and the amount expended therefrom shall be shown on the budget for the information of the taxpayers of the municipality in which the library is located. Moneys in such fund may be invested in accordance with the provisions of K.S.A. 10-131, and amendments thereto, with interest thereon credited to such fund. "

CAPITAL REVENUE.
All income budgeted for fixed assets or additions to fixed assets, except for the purchase of library equipment. Include income budgeted for sites, buildings, additions to buildings and furnishings.

Line 8.2a LOCAL GOVERNMENT.
Report all income sources for capital funds.

Line 8.2b STATE GOVERNMENT.
Report all income sources for capital funds.

Line 8.2c FEDERAL GOVERNMENT.

Line 8.3 CAPITAL EXPENDITURES.
Include funds for the acquisition of or additions to fixed assets such as building sites, new buildings and building additions, new equipment (including major computer installations), initial book stock, furnishings and equipment, regular purchase of library materials, and investments for capital appreciation.


PART IX: RESOURCES

Line 9.1 NUMBER OF BOUND VOLUMES.
Report physical units (example: volumes), counting items packaged together as one physical unit (examples: two audio or video cassettes packaged or checked out together). Smaller libraries for which counts of physical units are unavailable may substitute title counts. Except for the current volume, count unbound serials as a volume when the library has a least half of the issues in a publisher's volume.

Line 9.1a OWNED JANUARY 1.
The number of volumes and/or titles owned by the library at the start of the year. Inserted by Bibliostat Collect.

Line 9.1b ADDED DURING YEAR.
The number of volumes and/or titles added to the collection during the year through purchase or donation.

The number of volumes and/or titles "weeded" or lost during the year.

Line 9.2 NUMBER OF AUDIO MATERIALS.
Examples: audio CDs, audio cassettes.

Line 9.3 NUMBER OF VIDEO MATERIALS.
Examples: video cassettes, video discs.

Line 9.4 NUMBER OF CURRENT PERIODICAL SUBSCRIPTIONS.
Examples: magazines, newspapers, yearbooks, annual reports, proceedings.

Line 9.5 NUMBER OF MATERIALS IN ELECTRONIC FORMAT.
Report the number of items considered part of the collection, whether purchased or leased, that are designated to be processed by computer. Examples: CD-ROMs, U.S. Census tapes, locally mounted databases, serials and reference tools. Exclude library system software and microcomputer software used only by the library staff.

Line 9.6 NUMBER OF LOANS RECEIVED FROM SYSTEM ROTATING COLLECTIONS.
Examples: video cassettes, video discs, large print books.


PART X: INTERNET / ELECTRONIC ACCESS TO INFORMATION

Line 10.1 TOTAL NUMBER OF WORKING COMPUTER TERMINALS IN YOUR LIBRARY.
Report all the working computers now being used in your library, by both staff and public.

Line 10.2a INTERNET ACCESS BY PUBLIC.
Include computers that are used only by public.

Line 10.2b ACCESS TO YOUR ONLINE PUBLIC ACCESS CATALOG OR AUTOMATED CIRCULATION SYSTEM.
Include computers that access only your OPAC or automated circulation system.

Line 10.2c NUMBER OF USERS OF ELECTRONIC RESOURCES PER TYPICAL WEEK.
Include only computers that are used be general public.

Line 10.2d ANNUAL ESTIMATE OF USERS OF ELECTRONIC RESOURCES.
Multiply line 10.2c by 52.


PART XI: ANNUAL SERVICES

CIRCULATION TRANSACTIONS.
The total annual circulation of all library materials of all types, including renewals. Count all materials in all formats that are charged out for use outside the library. Count interlibrary loan transactions only for items borrowed and checked out to patrons. Do not include items checked out to another library. Do not use circulation multipliers. For example, if a film is checked out and shown to 30 people, count 1 circulation, not 30. Do not report "automatic renewals" as circulation. Report annual totals.

Line 11.1 CIRCULATION OF ADULT MATERIALS.
Include young adult materials.

Line 11.2 CIRCULATION OF CHILDREN'S MATERIALS.
Children's materials are those which are intended for use by persons age 14 and under regardless of the age of the person who checks out the material.

Line 11.3 TOTAL CIRCULATION.
Sum of Lines 11.1 and 11.2.

INTERLIBRARY LOAN TRANSACTIONS.
Transactions in which library material, or a copy of the material, is made available by one library to another upon request. It includes both lending and borrowing. The libraries involved in interlibrary loan are not under the same administration. Include items loaned to or borrowed from the State Library or the regional library system, including AV. Report annual totals.

Line 11.4 ITEMS PROVIDED BY YOUR LIBRARY TO ANOTHER LIBRARY. (ILL Lending)
Include materials or copies of materials in all formats. Exclude transactions between outlets under the same administration.

Line 11.5 ITEMS RECEIVED FROM OTHER LIBRARIES OR COMMERCIAL DOCUMENT DELIVERY SERVICES FOR YOUR PATRONS. (ILL Borrowing)

LINE 11.6 NUMBER OF CHILDRENS PROGRAMS.
Report total number of children's programs conducted.

Line 11.7 ATTENDANCE AT SUMMER READING PROGRAM, INCLUDING ADULTS.
Report the number of children and adults attending only Summer Reading programs.

Line 11.8 ATTENDANCE AT ALL OTHER CHILDREN'S PROGRAMS, INCLUDING ADULTS.
Report the number of children and adults attending all other programs for which the primary audience is children.

Line 11.9 TOTAL ATTENDANCE AT CHILDREN'S PROGRAMS.
Sum of Lines 11.7-11.8.

LINE 11.10 NUMBER OF ADULT PROGRAMS.
Report number of adult programs offered.

PART XII: SERVICES PER TYPICAL WEEK

Line 12.1 PUBLIC SERVICE HOURS PER TYPICAL WEEK.
Sum the number of hours open to the public per typical week for all outlets (main or central libraries, branches, and bookmobiles). Example: If a main library is open 40 hours per week and a branch is open 20 hours, report 60.

Line 12.1a ANNUAL ESTIMATE.
Multiply Line 12.1 by 52. Ignore minor variations in scheduled hours, but exclude extensive hours if the library is closed to the public due to construction, natural disasters, or other events, even if staff work.

Line 12.2 LIBRARY VISITS PER TYPICAL WEEK.
Report the total number of times individuals enter the library for whatever purpose (including attending activities and meetings and others requiring no staff services).

Line 12.2a ANNUAL ESTIMATE.
Multiply Line 12.2 by 52.

Line 12.3 REFERENCE TRANSACTIONS PER TYPICAL WEEK.
A reference transaction is an information contact which involves the knowledge, use, recommendation, interpretation, or instruction in the use of one or more information sources by library staff. Information sources include print and nonprint materials, machine-readable databases, catalogs and other holdings records, and, through communications or referral, other libraries and institutions and persons both inside and outside the library. Do not count directional transactions or questions of rules or policies. Examples of directional transactions are "Where are the children's books?" and "I'm looking for a book with the call number 612.3." An example of a question of rules or policies is "Are you open until 9:00 tonight?"

Line 12.3a ANNUAL ESTIMATE.
Multiply Line 12.3 by 52.

Line 12.4 NUMBER OF REGISTERED CARD HOLDERS.
Total number of patrons registered at your library.
