Interlibrary loan

CKLS Interlibrary loan record form (Microsoft Word)

11" by 8.5" sheet for recording interlibrary loan requests received and returned. Maintaining this sheet helps librarians track interlibrary loan items borrowed for their patrons.

CKLS Postage reimbursement form form (Microsoft Word)

4.5" by 8.5" sheet for requesting from CKLS headquarters photocopies of magazine articles. These requests can also be submitted via email.

CKLS Interlibrary loan photocopy request form (Microsoft Word)

8.5" by 11" sheet to record items lent to other libraries. When this sheet is filled, or early in January each year, mail this sheet in to receive reimbursement. CKLS only reimburses items lent to other libraries. CKLS does not reimburse postage for items borrowed from other libraries.

The ILL Annual activity form is filled out each January. The KIC Council Membership Agreement is filled out to join KICNET.
